This opportunity in a nutshell
What we need
Waitemata District Health Board (DHB) is seeking to appoint a Ministry of Health designated auditing agency (DAA) to complete a certification audit against the Health and Disability Services Standards NZS 8134:2008; this is to ensure that Waitemata District Health Board meets the required service standards for certification under the Health and Disability Services (Safety) Act 2001.
We want proposals which present a clear methodology and practical plan for conducting the certification audit and producing a clear Audit Report that reflects the requirements of DAAs in the Ministry of Health (MoH) Designated Auditing Agency (DAA) Handbook; Ministry of Health Auditor Handbook (revised 2016); For Health and Disability Services Standards NSZ 8134:2008.

Proposals from parties who are not Ministry of Health designated auditing agencies.
What’s important to us?
We are looking for a credible DAA that has strong proven capability, experience and resource to undertake a time and cost effective Certification audit in a large DHB. Our expectation is that the post-audit report is clear, providing clarity of any corrective actions and reasons for these; the report must also be delivered in a timely manner. Ideally we want the on-site audit completed in the final week of November 2016 or the first week of December 2016.
