Hauora Tairāwhiti is seeking to contract Service Providers to work co-operatively and in partnership to provide an enhanced Home and Community Support Services (HCSS) that delivers care support to people who are Māori aged 55 years or more, or 65 years (general population) or more, and who live within Te Tairāwhiti region.
This is primarily through co-operative, coordinated and integrated service of care that incorporates the older person’s natural, community, clinical and wider health professional supports. The focus is on providing a person-centred support service in a restorative and responsive model which balances goal aspirations and outcomes achievement. This model of care aims to improve, maintain and restore the capabilities of older people so that they can live well, safe and independently for longer in their homes.
We require Service Providers that understands the unique Tairāwhiti environment; the challenges and the opportunities for flexibility and innovative use of resources region wide. The intent is for a collaborative approach towards a person’s care planning, has an ability to use shared information IT platforms such as ‘international Resident Assessment Instrument’ (interRAI), and can demonstrate care service optimisation using clinical and health professional service delivery involvement.
