Northland District Health Board (NDHB) is issuing a Request for Proposals to provide Community Activity Programmes based in Kaitaia for Older People who live in the Far North who have been assessed and referred by the NDHB NASC service.
NDHB will purchase:
• A general community activity programme which may include older people who are frail, have a cognitive impairment and who can participate and benefit from such a programme; and
• A specialised dementia programme for older people who have a formal diagnosis of dementia and who require a targeted programme.
The service will comply with the Community Health, Transitional and Support Services – Community Activity Programmes Tier Level Two service specification.
This service will be operational by 1 July 2015.
