The Bay of Plenty region is the second largest of the Eleven Work and Income regions and is home to approximately 267,000 people. Our region is hugely diverse with sparsely populated rural communities and bustling urban city centres. Within the Bay of Plenty we have staff delivering services from the following service centres: Rotorua, Tauranga, Mount Maunganui, Bayfair, Greerton, Whakatane, Kawerau, Opotiki, Taupo, Turangi, Tokoroa, Murupara and Te Puke.
The target group for this service is Work and Income clients with a Health Condition, injury or disability – including mental health (HCiD) who have been on benefit 1 year or more, who have a willingness to work, but are limited in their capacity to look for, or be available to work because of a diagnosed health condition(s) including mental health, that can be treated effectively in a primary care setting.
The Provider will be responsible for the case management of participants to support them to achieve and sustain employment outcomes that align with their individual goals and benefit obligations.
The Provider will carry out an assessment for each client, to identify their goals, strengths, skills and challenges. Once the client’s enrolment is complete they will become a participant of the Journey to Wellness service. In conjunction with the participant, the Provider will develop a plan to staircase the participant into sustainable employment.
The Provider will develop and maintain positive working relationships with existing services and organisations, including government agencies, community organisations, health and disability support providers, family and whanau, and will facilitate access to other services that can provide support for participants to successfully achieve and retain employment
It is expected 50% of client accepted on to the programme will obtain employment.
