Overview

Older people are at risk of feeling lonely, socially isolated, and/or invisible. This can have a negative impact on their mental and physical wellbeing.

This Proof of Concept (PoC) takes a holistic approach to understanding an individual’s needs and requirements that underpin their sense of wellbeing, beyond management of medical conditions and treatment plans.
The decline in older people’s health and wellbeing often occurs in the home, independent of any engagement with the health system. A consequence of this is that older people may need to access hospital/aged-related residential care (ARRC) as the system did not know their health was deteriorating, so no appropriate interventions were made. This inability of the health system to engage in early/proactive interventions can have a serious negative impact on patient’s long-term health outcomes, and a significant cost impact to the health and disability sector.

It is well known that people often have better health outcomes and a greater level of wellbeing when they are able to stay within their own environment.

The Home Sensors PoC aims to understand the ways in which sensor technologies can promote proactive care and help older people improve their wellbeing and extend the time in which they can maintain independence and be active in their communities. Whilst we are specifically focusing on environmental sensors that capture non-health data, we acknowledge that some offerings are a combination of sensors (including those that capture health data), that together can provide a broader picture to help inform healthcare decisions and/or interventions.

Currently the information collection and service integration part of this process has not been tested in an NZ DHB or through the PHOs and communities’ organisations that provide care. As a result, older people living in their home environment may not be receiving the care that could be provided if the technology was used to best effect.
