MOBILITY ACTION PROGRAMME (MAP)

In mid-February 2016, Registrations of Interest (ROI) will be invited for Tranche 2 of the Mobility Action Programme (MAP).

The aim of the Mobility Action Programme is for people with musculoskeletal health conditions to fulfil their health potential and increase independence. This will be achieved through improved access to high quality advice assessment, diagnosis and treatment including education programmes to encourage self-management and rehabilitation programmes to improve function, activities and participation.

The Mobility Action programme will part fund the development, implementation and evaluation of community based models of care that improve access to evidence-informed, multidisciplinary care and clinical outcomes for people with musculoskeletal health conditions.

The principles of the MAP are that:

a) Treatment and care respects consumer’s wants, needs and preferences, consumers have access to the education and support needed to make decisions and participate in their own care.
b) People with musculoskeletal health conditions should have access to early, multidisciplinary, evidence informed advice, assessment, diagnosis and treatment including education and rehabilitation by appropriately qualified practitioners.
c) Models of care should be relevant and customised to the local environment.
d) Care should be well coordinated across the various providers and services that people use.
e) Disparities in access to care and health outcomes should be identified and reduced.
f) Information will be collected and analysed by Mobility Action Teams that enable assessment of patient experience, clinical outcomes and value for money of the services that have been delivered.

It is expected that proposals for effective musculoskeletal care will include improving access to:

g) supported self-care
h) informed and accessible primary care services, and
i) Expert multi-disciplinary community services.
