The Te Waipounamu region is transitioning to a single, bulk-funded model for Home and Community Support Services (HCSS), replacing the fragmented, former District Health Board (DHB) specific arrangements. This opportunity is also to develop an innovative approach to individualised funding within a clinical framework. This shift aims to address issues such as inequities in access, inefficient service delivery, inconsistent wait times for clients, varying assessment processes and the administration burden on our providers.
We are looking to contract with a small number of providers to supply HCSS and IF throughout Te Waipounamu who understand the restorative model of care and can apply it in accordance with the latest version of the National HCSS Service Specifications.
Expected outcomes when transitioning to a standardised regional bulk funding model:
- Improve Equity of Access
- Promote Better Health Outcomes
- Support Hospital Flow
- Reduce Wait Times for Needs Assessment
- Optimise Health NZ Commissioning Resources:
- Encourage Workforce Development and Innovation:
- Improve Monitoring of Quality and Outcomes
- Clinically supported self-management
This initiative offers a partnership model that prioritises outcomes, promotes innovations in service delivery, and supports the development of the HCSS workforce.
