What we need is a Community Podiatry Service that is an integrated programme across the community with a single point of entry to the service based on consistent risk assessment and stratification. This service will manage all high risk patients who meet the eligibility criteria.
We wish to select one Lead Provider who will manage the single coordination service, provide the majority of podiatry services and ensure consistent best practice is applied.
Providers may elect to be a “service deliverer” only. The arrangement between the service deliverer will be negotiated between the Lead Provider, service deliverer and Hawke’s Bay District Health Board. Service delivers are required only to “Register an Interest” in delivering the service at this time.
