Counties Manukau Health is developing a new system of care for primary and community services to support people and whaanau living with Long-Term Conditions (LTC). This system of care will be co-designed with service users, their whaanau and delivered in partnership between primary and community sector, and the District Health Board (DHB).
It will see the development of comprehensive, culturally-capable services delivered across the Counties Manukau rohe. The aim is for a system of care that will enable people in our communities to access a responsive, integrated range of health and social services close to their homes and their whaanau. People will see their culture and what matters to them reflected in the care and support that they receive.
The driver for this change is the need to do more to meet the needs of our whaanau with Long-Term Conditions. We need to ensure equity for our population, with people supported to live longer, healthier lives. Our communities have told us that we need to do more and that we need to do things differently; this message has been strongly echoed by the findings and recommendations recently produced in the LTC Discovery Phase Report - https://countiesmanukau.health.nz/assets/About-CMH/Performance-and-planning/integrated-care/CMH-Health-Co-design-Discovery-Phase-Report-May-2019.pdf
We want to work with a broad range of community-connected, culturally-capable, passionate and enterprising providers of primary and community care; providers who put service users and their whaanau at the centre, and who have manaakitanga at the heart of everything that they do. In particular, we are looking for partner organisations to work on co-designing and implementing new models of care for Maaori and Pacific People and people living in Quintile 5 areas with two or more long-term conditions (target population).
