The BOPDHB is seeking proposals for the delivery of responsive Home and Community Support Services for eligible clients.
A responsive Home and Community Support Services is about delivering a flexible home based service that is responsive to individual need, thus empowering older people with their informal carers to take control of their conditions, share in the decision making of their care and support them in maximising their independence.
We are looking for credible providers who have the capability, experience and infrastructure to deliver the services identified in this request for proposal. They need to have a good track record in the management and implementation of these or related health services, with the ability to form effective networks and strong relationships within our local community.
